A flipbook used at a 5-foot (1.5 meters) screening distance with single surrounded LEA SYMBOLS®. This chart meets the preferred practice recommendations from the National Expert Panel to the National Center for Children’s Vision and Eye Health at Prevent Blindness.
Designed for use by lay screeners, nurses, and others who screen children in Educational, community, public health, or primary health care settings.
Each card contains a single LEA SYMBOLS® optotype surrounded by a 50%-spaced crowding bar. The attached measuring cord ensures a 5-foot (1.5 meters) screening distance between the flipbook cards and the child’s eyes.
The EyE Check Screener is available in 2 versions: one for children ages 3 years and one for children ages 4 and 5 years. The EyE Check Screeners are also available as a package.
#252100 – 20/40 critical line screening for children ages 4 and 5 years. The flipbook begins with four 20/80 cards to familiarize the child with the screening task and continues with eight 20/40 cards for screening each eye monocularly.
